OC-061 IMAGE-BASED DEEP LEARNING MODELS (DLMS) IDENTIFY ABDOMINAL WALL RECONSTRUCTION (AWR) PATIENTS RECEIVING PREOPERATIVE BOTULINUM TOXIN A (BTA) WHO REQUIRE COMPONENT SEPARATION TECHNIQUE (CST)

چکیده
Abstract Aim Preoperative BTA facilitates muscle/fascia elongation and fascial closure, which increases the durability of hernia repair may reduce requirement for CST. The aim this study was to develop image-based DLMs that predict whether patient will require CST if injected with prior surgery. Methods An institutional database used identify AWR patients who received preoperative had CT imaging. Axial cuts hernias were rendered train a DLM. primary outcome ROC predicting DLM tested on scans from pre-identified subset underwent did not receive BTA. Results There 116 met criteria (4,580 images). Of these patients, 69 (59.5%) required (2884 images); 47 (40.5%) undergo (1,696 0.78 (Figure 1); accuracy, sensitivity, specificity 0.79, 0.86, 0.68, respectively. 98 in test set; 57 (58.1%) predicted 41 (41.9%) not. For an M1 component, 21.1% be spared versus 46.8% those M2-M5 component (p=0.04). Figure 1 Model Performance Prediction Conclusions Image-based accurately receiving When applied set, very likely CST, is consistent previously reported data, further validating model.

Botulinum Toxin A as an Adjunct to Abdominal Wall Reconstruction for Incisional Hernia
BACKGROUND Repair of large incisional hernias remains a surgical and costly challenge. Temporary paralysis of the lateral abdominal wall muscles with topical administration of botulinum toxin A (BTA) is a new therapeutic concept, which may obviate the need for component separation technique (CST) for repair of large incisional hernias. Current literature on the administration of BTA as adjunct ...
متن کاملAbdominal Wall Reconstruction Utilizing the Component Separation Technique: Does Reinforcing Mesh Reduce Recurrences?
Purpose: The component separation technique was initially described as an autologous tissue transfer without biomaterials. This study evaluates the results of component separation hernia with and without reinforcing mesh. Methods: We retrospectively reviewed 126 consecutive component separation hernia repairs at a single institution between 2004 and 2009 and compared preoperative characteristic...
متن کاملThe Component Separation Index: A Standardized Biometric Identity in Abdominal Wall Reconstruction
OBJECTIVE Reconstruction of traumatic ventral hernias often requires additional techniques to the abdominal wall component separation, such as the use of interpositional reconstruction with an acellular dermal matrix or other mesh to bridge the defect. METHODS We have developed a new value termed the "Component Separation Index" to evaluate ventral hernia defects. Choosing a fixed point on a ...
متن کاملTransversus abdominis muscle release: a novel approach to posterior component separation during complex abdominal wall reconstruction.
BACKGROUND Several modifications of the classic retromuscular Stoppa technique to facilitate dissection beyond the lateral border of the rectus sheath recently were reported. We describe a novel technique of transversus abdominis muscle release (TAR) for posterior component separation during major abdominal wall reconstructions. METHODS Retrospective review of consecutive patients undergoing ...
متن کاملDiscussion: Minimally invasive component separation with inlay bioprosthetic mesh (MCSIB) for complex abdominal wall reconstruction.
C abdominal wall reconstruction requires answering two independent and related questions: What is the best way to treat the abdominal wall, and what is the best way to treat the skin? Butler and Campbell present their solutions to these two problems in their article entitled “Minimally Invasive Component Separation with Inlay Bioprosthetic Mesh for Complex Abdominal Wall Reconstruction.” Althou...
